Request Return Material Authorization (RMA)

Do You Need Service or Repair?

Before returning any products to Guided Wave, for service or repair, you must receive a return materials authorization (RMA) number first. To receive an RMA number, please complete and return the RMA form. Once submitted you will be contacted within 24 business hours (Monday – Friday).

Before we begin the repair, GWI will contact you with a price quote and an estimated repair or replacement delivery date.

PLEASE NOTE:

  • Return shipping from GWI is pre-paid and add by Federal Express.
    • Domestic is 2 day service.
    • International is 3-5 day service.
  • If you require a different shipping method please provide your account numbers, freight forwarder and/or any other shipping contact information at the time of return.
  • MSDS form is required when returning a probe or flowcell.
